Paliperidone is an atypical antipsychotic medication commonly used to treat schizophrenia, schizoaffective disorder, and bipolar disorder. It a metabolite of risperidone has similar mechanism action, primarily blocking dopamine 2 receptors (D2 receptors) in the brain. various adverse effects, including extrapyramidal symptoms, weight gain, metabolic disturbances. Catatonia rare but severe side effects can occur context underlying psychiatric, neurologic, or general medical condition. Paradoxically, antipsychotics for treating schizophrenia spectrum disorders precipitate worsen catatonic symptoms. The report suggests that 17-19% all cases diagnosed as catatonia due other conditions are medication-induced. neuropsychiatric syndrome presents cluster psychomotor signs symptoms resulting movement behavior aberrations. Various mutism, stupor, rigidity, abnormal movements, characterize catatonia. potentially life-threatening condition requiring prompt recognition management. Here, we present case patient with associated long-acting injectable paliperidone intramuscular therapy schizophrenia.
